According to the San Francisco Chronicle, 49ers safety Michael Lewis suffered his third concussion of the 2009 season after attempting to make a tackle with his head down against Falcons RB Michael Turner:

Lewis picked up his first concussion of the season Aug. 18 in a joint practice session with the Raiders in Napa. Eleven days later, a random knee to his head in the Dallas exhibition game led to concussion No. 2.
The 49ers do not return to action until Oct. 25, so the team medical staff will have more time to see if Lewis is fit to play. However, with the recent study published by the NFL about the increased potential for dementia in players who suffer multiple concussions, expect the 49ers to take it slow with Lewis.
