At the end of the Tour de France’s Stage 7, the 1KM banner -- a big, inflatable semi-octopus of a sign post -- lost air and flopped onto the course:

All the riders involved were given the same stage time relative to position, so no harm done there. As far as actual harm, it looks like there were no serious injuries, although Adam Yates came away a little bumped and bruised:

According to ASO a fan accidently got his/her belt caught on a cable that powered the generator that held up the inflatable banner.
