San Francisco 49ers quarterback Alex Smith has not been cleared to play on Monday night against the Chicago Bears after suffering a concussion in Week 10 against the St. Louis Rams, a source tells ESPN's Chris Mortensen. Smith practiced on a limited basis during the week and was thought to be on track to play. However, Smith was not cleared by a neurologist, clearing the way for Colin Kaepernick 's first career start.

Kaepernick finished with 117 passing yards last week against the Rams after taking over for Smith in the second quarter. He also rushed for 66 yards and a touchdown.
Fantasy Impact: Fantasy owners who were counting on Smith for Monday are likely out of luck for this week. Things had sounded promising for Smith earlier in the week but nothing is a sure thing when it comes to a concussion.
If league rules allow, desperate fantasy owners should grab Kaepernick or Chicago's Jason Campbell, who is starting in place of Jay Cutler (concussion). Otherwise, neither have much fantasy value.
